"A Farewell to Arms" American in the Italian army

"A Farewell to Arms" American in the Italian army?
Why was it such a big deal that an American was in the Italian Army in WWI? In the first book an English Nurse asked him why he didn't join the English instead of the Italian army. Why? I thought that Italy was part of the Allies just like the U.K so what was the difference?

As the American government proclaimed itself neutral when the Great War broke out in 1914, reports of the atrocities perpetrated by German and Austro-Hungarian troops against other neutral nations such as Belgium and the netherlands prompted many young Americans to volunteer for non-combat roles in any Allied Army who would accept them. Another example of this American volunteering were also the famous "Flyng Tigers", a group of American pilots who in 1937 formed their own squad within the Chinese Air Force during the war with Japan.
